- Online başvurunuz e-postayla doğrudan okulunuza gönderilecektir. Başvuru sürecini tamamlamak için okulunuzun verdiği tüm talimatlara uymak sizin sorumluluğunuzdadır. Option 1: Submit proof of the Meningitis ACWY vaccine was taken within 5 years of enrollment, and after student’s 16th birthday is required.
Option 2: Submit proof of a complete 2 to 3 dose series of the Meningitis B vaccine was taken.
Option 3: Submit a declination form (See below.), waiving the meningitis vaccination.
