Please Read Carefully Before You Apply
- Your application will be sent directly to the program via email. It is your responsibility to follow the instructions provided by the school in order to complete the application process. Applicants are advised to send questions directly to the school admissions office to ensure the quickest response.
- Some courses may have an application deadline. You are strongly advised to submit your application well before the course start date to ensure there is adequate time for processing.
- When submitting your application, prepare to send a copy of your passport to the school. You may also be required to pay a tuition deposit and submit additional documents. Your program may also require you to submit forms with your signature for security purposes. Please follow the instructions from your program carefully in order to complete the enrollment process.
- You must obtain a student visa in order to legally study abroad and enroll in your chosen English program. When applying for your student visa, you will submit the required forms received from your school, and other documents, to the designated embassy or consulate. We encourage you to apply to your school well in advance of your start date, to give you sufficient time to have your student visa processed.

Copy of Passport (I.D. Page)
Instructions:
Make a clear, readable copy of your passport biographic page containing your name, birth date, expiration date, and the bottom two lines filled with letters and numbers.
Bank Statement
Instructions:
UCLA will accept bank documents and letters in English dated within 6 months of your submitted Form I-20 application. All supporting bank documents or letters must show available funds to meet or exceed the amount of your program.
Financial Sponsor Statement Form
Instructions:
If anyone other than the student will be listed on the accompanying financial documentation submitted as proof of financial ability, please fill out the personal information on the form below, and indicate relation to the student. If funds exist in the student’s name, please mark “Self” below.

English Test Score
Instructions:
Student participating in courses offered solely by UCLA Extension must meet the English Language Requirements outlined below.
Scores that are equal to or higher than the listed minimum scores from the following tests, taken within the past two years, can satisfy the English proficiency requirement.
TOEFL: 80 (iBT or Home Edition)
IELTS: 6.5
iTEP: 4.5
CAE: B (CEFR C1)
CPE: C (CEFR C2)
TOEIC: 685 (Listening & Reading or Speaking & Writing)
DUOLINGO (DET): 105
